For Patients with Medicare Part D Plans
Beginning in 2026, if you have Medicare Part D coverage, your maximum
out-of-pocket costs for all your covered prescriptions, including LEQEMBI IQLIK, will be $2,100 per year. You may also choose a Medicare payment option that lets you divide those costs into monthly payments throughout the year, which may help make expenses more predictable and easier to manage. Once you reach $2,100 in out-of-pocket expenses, your covered prescription medications will cost $0 for the remainder of the year. The out-of-pocket cost cap is updated annually.

Medicare Prescription Payment Plan
If you have a Medicare drug plan or a Medicare Advantage plan with drug coverage, you're eligible to opt in to the Medicare Prescription Payment Plan. The Medicare Prescription Payment Plan is a free budgeting tool for your prescriptions. It can help manage your prescription costs by spreading payments over the calendar year (January-December). Enrollment is flexible, allowing you to opt in at any time each year.

For patients with employer or private health insurance, your out-of-pocket cost for medicines is determined by your insurance provider.

What to keep in mind

What you'll pay for LEQEMBI depends on a number of different factors such as:

-Your insurance plan

-Deductible status and where you are in your plan year

-Other healthcare expenses

-Where you receive treatment

-Whether you qualify for financial assistance

What you pay may also change over time. When you start treatment, costs will be based on receiving LEQEMBI infusions twice monthly (once every 2 weeks) or 2 once-weekly self-injections with LEQEMBI IQLIK. After 18 months of treatment, costs may change if you start maintenance treatment. You and your healthcare provider will determine if you can switch to less-frequent, once-monthly (once every 4 weeks) infusions, or once-weekly (once every 7 days) at-home injections with LEQEMBI IQLIK. LEQEMBI IQLIK is an injection given under the skin (a subcutaneous injection) by you or a care partner.

How costs are calculated

The list (or wholesale) price for LEQEMBI intravenous (IV) infusions is $271.83 for a 200-mg vial and $679.60 for a 500-mg vial. Because LEQEMBI IV infusions has weight-based dosing, the price per patient is based on the number of medication vials needed. For example, based on the list price, treatment for a patient who weighs 165 pounds and receives LEQEMBI IV twice monthly (once every 2 weeks) would cost $28,270.32 per year.

The list price for LEQEMBI IQLIK injections is $770 for two 250-mg, single-dose, prefilled autoinjectors; and $385 for a 360-mg, single-dose, prefilled autoinjector. Because the starting dose of LEQEMBI IQLIK is 2 injections, administered once weekly for the first 18 months, treatment would cost $60,060 for the first 1 and a half years. After 18 months, if your HCP decides maintenance dosing is right for you, LEQEMBI IQLIK is 1 injection administered once weekly making treatment $20,020 per year.
